This can save the patient and the health plan money.

While 皇冠官方APP下载 is offered at no cost to most eligible patients, some patients may have a co-pay similar to when they see their primary care provider due to the relationship established with their health plan.

皇冠官方APP下载's services are designed to care for the whole patient. Our interdisciplinary clinical teams address the physical and mental needs of our patients, along with social services. Patients can expect to receive the type of care found within primary care and urgent care, including but not limited to the following examples.

• In-home lab, X-rays and ultrasounds
• IV and injectable medications, including antibiotics for serious infections
• Prescriptions and medication review
• In-home pharmacist, dietitian and social worker
• Referrals for home oxygen, durable medical equipment, home health services and hospice
• Palliative care consultations
• Catheter insertion and removal
